After filling out this form, you may pay your $50 deposit online (credit card only), or contact the office for other arrangements. Once the deposit is received, you will be mailed our "Informed Consent and Liability Release Agreement" (which must be signed and returned via mail or fax) and the "Mantario Summer Program Equipment List". PLEASE NOTE: A $50 deposit is required at the time of registration and full payment is required 14 days prior to the trip. We will refund a deposit up to 14 days prior to the departure date, minus a $15 administration fee. Deposits are forfeited for cancellations made less than 14 days from departure date.
